POLST Resources Available for Physicians, PAs and Patients

Physician Orders for Life Sustaining Treatment (POLST) is a medical order intended to improve the quality of end-of-life care by giving people with serious illness or frailty more control to direct the kinds of treatment they receive.

The POLST Paradigm is based on effective communication of patient treatment wishes and documentation of medical orders—on a brightly colored form signed by a physician, physician assistant, or nurse practitioner—after a discussion with the patient or their health care representative.
