It is OMA policy to endorse adoption of the “Suggested Exam Protocol for the Physician,” “Model Physical Exam Form,” “Model School Sports Pre-participation Examination” and “Model School Sports.”

Pre-sports physical examinations for participation in school sports activities should be required by schools at Grade 7 and 10, or upon entry into the school system.

The Model Pre-participation Annual Interval History should be completed by parents of the athlete in each year a physical examination is not required prior to sports participation.
