Standardized Insurance Cards

The OMA recognizes the need for insurers to standardize insurance cards with effective dates, primary care provider, referral requirements, copay amounts, network, list of insured under subscriber, Group and ID numbers, and be made on printed material; and that a universal format also be applied in the same manner to explanation of benefits.
