Early Discussion and Resolution After an Adverse Event
We know that patient safety is a priority of physicians and physician associates in Oregon. When there is an adverse event it can feel overwhelming for patients, families, and the entire care team. There are resources available to help medical practices navigate the process. The Oregon Patient Safety Commission, in collaboration with partners such as the Oregon Medical Association, supports the continued development of Oregon's Early Discussion and Resolution Program.
What is the Early Discussion and Resolution Program ?
Early discussion and Resolution Program (EDR) is a state program that allows members of the care team to talk with the patient or patient's family about the facts and circumstances that led to a serious adverse event. When members of the care team are given the opportunity to acknowledge the experience and explain how they will learn from it and what changes they will make going forward, it benefits everyone. EDR is a process that occurs outside the legal system and addresses what patients most often want: transparency, information, and sometimes even just an apology. EDR does not replace other processes that occur with adverse events; it is an additional tool.

When Should I Utilize the EDR?
Although physicians and physician associates do their best to ensure patient safety, adverse events to occur. Often negative outcomes or harm to patients occur through no fault of any one person. EDR can benefit the patient, their family and the care team by creating a shared understanding. In essence, EDR humanizes the experience for everyone while helping to identify changes that will occur going forward. Ensuring that you and other clinicians in your organizations are aware of the EDR is a good start.
If you are involved in an adverse event it is important that you follow protocols set forth in your organization. It is also important to contact your professional liability carrier for guidance. Reaching out to the EDR Program at the Oregon Patient Safety Commission is an additional step than can enhance the other processes.
